Paris-based Relaisante has raised €6m in financing co-led by Ternel and Aquiti, with participation from Elaia and AFI Ventures. The investors describe the transaction as a seed round; the managed register retains Tech.eu's approved Other-round classification.
Relaisante helps hospital teams find available home-care professionals and coordinate a patient's transition after discharge through a single request. Its network connects more than 315 healthcare facilities with about 12,000 home-care professionals, and the company reports an average of 18 minutes to identify providers able to take over a patient's care. The financing will support broader coverage and a target of more than 1,000 connected facilities by the end of 2027.
Making discharge coordination a shared workflow
A medically ready patient can remain in hospital when staff cannot quickly confirm suitable care at home. Relaisante is turning that fragmented search across nurses, therapists and equipment providers into a networked workflow with visible availability.
Scale will depend on more than adding facilities. The platform must maintain reliable provider coverage by geography and care need while fitting clinical handoffs and data-protection requirements. If Relaisante can preserve its response time as the network grows, it can reduce administrative work for hospitals and uncertainty for patients and families.
